I can't read Japanese! Mind helping me out?

It's embarrassing how badly I need to learn the Japanese language. Who here can help me out with a downloader? I can't cheat with copy/paste to google translate haha. Just needa know what goes in what boxes.

Password (I can't read kanji, but I think it's safe to assume it's something saying to confirm your password)
